Responsibilities
- Conduct primary assessment, diagnosis and treatment planning for acute and chronic conditions
- Manage patients across the full primary care pathway including preventive care
- Prescribe and oversee pharmacological and non-pharmacological treatments per guidelines
- Coordinate referrals to specialists and ensure timely handover
- Document clinical assessments and maintain accurate records
- Participate in multidisciplinary meetings and quality improvement initiatives
- Support infection prevention, medication safety and resource stewardship
- Assist in service coordination and patient flow optimization
